The Nurse Practitioner will manage delegated medical aspects of care in collaboration with the supervising physician and operate within approved written protocols in accordance with the State’s Nurse Practice Act.  Demonstrates critical thinking skills, scientific judgment and leadership in the anticipation and planning of the care of the patient.  Possesses excellent organizational skills and verbal and nonverbal communication skills.  The NP remains accountable for all actions to the supervising physician who has agreed to practice in collaboration with the NP and is authorized to delegate those medical acts agreed upon in the approved written protocols and who is accountable through the normal channels set up within the Bylaws, Rules and Regulations.

Palliative Care Nurse Practitioner (steward center) SUMMARY: The Palliative Care Nurse Practitioner, under clinical supervision of a licensed physician, is responsible for diagnosis, treatment and management of patients who are dealing with an incurable progressive illness and provide care in keeping with parameters established for nurse practitioners. As a primary care provider, the Nurse Practitioner will provide services appropriate for a palliative care specialist. Essential job responsibilities: Under the guidelines and protocols established for practice and specialty, performs patient assessments and develops a plan of care for patients with symptom control problems. Collaborates with the Medical Director, Team Physician, and attending physician as appropriate, in managing medical, psychosocial needs of patients. Initiates referrals to physician specialists and other providers as needed. The Palliative Care Nurse Practitioner may provide hospice and/or palliative care services in a variety of settings in hospitals, home care, and assisted living while consistently demonstrating the Hospice Savannah competencies and values. The Palliative Care Nurse Practitioner is accountable to and evaluated by the Medical Director. Input is also sought from other members of the IDT. QUALIFICATIONS: Masters Degree in Nursing. Successful completion of an accredited nurse practitioner program in specialty of family practice / adult health/ pediatrics (as required to perform duties). Experience in the care of terminally ill patients as defined by: three years nursing experience with two years experience in hospice palliative/ care-setting with demonstrated experience in the principles of pain and symptom management. Ability to work successfully within an interdisciplinary team (including volunteers) toward positive outcomes for patients and their families. Ability to represent HSI within the physician community as well as general public with excellent communication and presentation skills.
